I am writing on behalf of my late uncle, who passed away on May 19, 2026, to respectfully request a copy of his Certificate of Employment. Unfortunately, the only employment-related document currently in our possession is his Company ID bearing Employee No. 02-024.

We need this document as part of the requirements for processing his death benefit claim with the Pag-IBIG Fund.
